1. Which category or categories reflect the candidate's contributions to the industry? *

2. Has the candidate been involved in the industry for at least 10 years? What years did the candidate work in the industry? When did the candidate retire or leave the industry, or is the candidate still involved in the industry?

3. In what positions was the candidate employed during that period? Indicate significant positions during the period and include a short bio or resume if possible.

4. What were the most significant accomplishments of the candidate? What is the single most important contribution the candidate made to the industry (in 500 words or less)?

5. Why is this candidate different from other candidates that may have made similar contributions to the industry?

6. How has the candidate exhibited leadership in the industry both personally and professionally? Was the candidate involved in any industry associations or industry organizations? If so, for how long? Did the candidate hold leadership roles in the organization(s)?

7. Has the candidate been presented with any industry awards or been recognized for industry achievements?

8. What is the candidate’s current age and health status?

9. To the best of your knowledge, will the candidate personally be able to attend the 2026 award ceremony if selected? The event is tentatively scheduled for Fall, 2026 in Washington, DC.

Please note: If a candidate is identified as a potential honoree but is unable to attend or participate in the Wireless Hall of Fame awards dinner, that candidate will remain in the nomination pool and be reconsidered the following year. Since the Selection Committee is comprised of different members each year, there is no guarantee of reselection by a subsequent committee.
